Free Advance Directives Available for Alaska Residents

Advance care planning is the best gift you can give to the people you love. Yet only one-third of all adults have documented their healthcare wishes.

Advance care planning allows people to communicate their values, goals, and preferences for their future medical care and to prepare themselves and their loved ones for medical decision making. Efforts to encourage such planning have faltered in the past because many people feel intimidated or confused by complicated legal advance directive forms and many clinicians lack the training, time, and resources to have these discussions. Alaska residents can now go to and get an advance directive form developed specifically for Alaska, in both English and Spanish.

This no-cost form is legally binding and easy to use. Anyone can complete the process on their own by downloading the Alaska advance directive here.  No email address or name is required to use this valuable resource.

The Prepare for Your Care Website


The website and easy-to-read advance directives have been shown to improve documentation of healthcare wishes in the medical record by up to 35 percent. was developed by Dr. Rebecca Sudore, a professor and geriatrician at the University of California, San Francisco, based on extensive research with older adults and diverse populations. The site has many resources available in both English and Spanish, including video stories that guide people through preparation for future medical care and completing an advance directive.
